The following services were sought and awarded through the Combined Funding Application of the Montgomery County Human Services Planning and Development, United Way of the Greater Dayton Area, and the Montgomery County Frail Elderly Services Advisory Committee.







United Way Award

Alzheimer's Disease & Related Disorders Association

Delivery of a comprehensive dementia community-model for seniors, caregivers and the community to increase education, care & support, counseling and use of family goal setting.



Catholic Social Services

Senior Outreach Program provides education, assessments, person centered case management, respite, and visiting assuring seniors safety and quality of life in the least restrictive environment.



Dakota Center

Dakota Center Golden Agers Program serves seniors 60 yrs. & older living in Dayton and Montgomery County with health education/nutrition/and community resources for self sufficiency.



East End Community Services

Senior Outreach Services will allow frail and elderly citizens of East/North Dayton to live independently with the help of case management and linkage services.



Goodwill Easter Seals MV

Easter Seals Adult Day Service is a safe, engaging and caring day program for seniors while family caretakers are working or attending to other responsibilities.



Legal Aid of Western Ohio

LAWO's Senior Legal Services program helps frail elders maintain independence through outreach, legal education classes and clinics, prevention, crisis stabilization, and extended legal services.



Life Essentials

Life Essentials provides guardian-of-person services to frail elders in Montgomery County. This legal process is utilized when a person lacks capacity regarding health or safety.



Reach Out Montgomery County

Reach Out provides elder medical care in its clinics, at its pharmacy, and at the senior's home with community paramedic visits.



Rebuilding Together Dayton

NeighborCare assists low-income elderly Montgomery County homeowners to age in place safely with the installation of modifications, like step in showers, grab bars and handrails.



Senior Resource Connection

Provides nutritionally balanced, hot meals served in a restaurant style setting to 60+ individuals promoting healthy eating habits, resistance to illness and enhancing social interaction.



Senior Resource Connection

To provide home delivered, nutritionally balanced, hot noontime meals to meet the frail elderly unmet nutritional needs, promote self sufficiency and prevent premature institutionalization.



Senior Resource Connection

The Visiting Home Care Program of the Senior Resource Connection delivers in-home personal care and homemaking services to frail elderly and disabled in Montgomery County.



Senior Resource Connection

Representative Payee program certified by the Social Security Administration works with Adult Protective Services in handling the financial affairs for residents age 60 and older.



Senior Resource Connection

Registered Nurse completes in person comprehensive assessment for at risk Montgomery County residents age 60 and over. Education, information, referrals provided for individual needs.



Senior Resource Connection

To provide emergency home delivered, nutritious meals to meet the needs of frail elderly after injury/hospitalization or while awaiting entry on other subsidized meal programs.



Wesley Community Center

The Wesley Center Seniors Program provides individuals 50 and over with case management, personal/medical assistance transportation, activities, and intergenerational exchange to help seniors maintain independence.
