2022 Wellness Program Incentives Appeal

In an effort to ensure that all eligible employees have the opportunity to fully participate in the incentive program, we are providing an Appeal Process for employees to submit an appeal if you believe your incentives were not properly awarded through no error of your own.


Things You Need to Know Things You Need to Do Resources
  • The deadline for filing an appeal is March 31, 2023.

  • All Appeal information will remain confidential.

  • Timing of the final decision will vary based on the complexity of the issue.

  • You will be contacted by the committee with a decision.

  • All Appeal decisions are FINAL.

  • Download a Benefits Appeal Form.

  • Complete the appeal form in full, provide a statement detailing the reason for your appeal, and attach supporting documentation. Documentation would include, but is not limited to: fax confirmation pages or documents printed from the web site showing dates of completion for activities.

  • Please do not send confidential medical information to Human Resources. These documents should go directly to the YMCA.
