NOTE: You only need to enroll during annual enrollment if you want to make a change to your benefits plan or if you want to participate in a flexible spending account for 2022. If you do not enroll during annual enrollment, your benefits will remain the same with the exception of the flexible spending accounts.

All forms including verification forms and dependent eligibility documentation for newly-added dependents (View dependent eligibility) must be received by the Benefits team by 5:00 pm on October 22, 2021. Forms can be submitted via fax at (937) 496-7407, email at hr@mcohio.org, or by drop off at the Administration Building, 9th floor.
