Complete the application for the work you or your company will be doing. All applications must be properly completed and signed. 1. Fill in information and print the application form 2. Sign application 3. Scan application and supporting documents/drawings and send as an email attachment to Permits@mcohio.org, or Fax to 937-781-2686 4.
