Connection Billing Information Forms must be filled out for ALL New Water & Sewer Services.
Required Fields appear in light gray. 

New Connection Billing Information Form will now auto submit in an email from you without having to save, scan, and attach in an email to permits@mcohio.org

Permit Applications are fill in format and still need to be saved, scanned and emailed to permits@mcohio.org

Fill In Application Instructions:

Complete the application for the work you or your company will be doing.
All applications must be properly completed and signed.
1.  Fill in information and print the application form
2.  Sign application
3.  Scan application and supporting documents/drawings and send as an email attachment to Permits@mcohio.org, or Fax to 937-781-2686
4. Documents/drawings that can not be scanned or faxed may be mailed or delivered to
