How to apply for Financial Assistance:
If you are applying for financial assistance, you must ensure you have all the necessary documentation before you arrive for your appointment. If you do not have all the necessary documentation listed below, your application (and subsequent approval or denial of funding) will be delayed or denied. During your appointment, the financial assistance specialist may determine that additional documentation is required to determine eligibility for assistance.
If you have a payee for Social Security or Veterans Administration Benefits, your payee must come with you to the application appointment.
YOU MUST PROVIDE THESE DOCUMENTS AT THE TIME OF YOUR APPOINTMENT

- Military discharge papers (DD-214 or equivalent)
- Proof of residency in Montgomery County, Ohio for 90 days. The proof of residency must have the name of the veteran or spouse, address, and date clearly listed. The proof cannot be handwritten. Examples of residency proof could include a bank statement, utility bill, or official envelope with the above items clearly displayed. The Statement of Residence, Household Composition, and Cost (Landlord Statement) form is not acceptable as proof of residency
- Marriage Certificate, Notarized statement of separation
- Most recent bank statements for checking and savings accounts
- Birth Certificates for all claimed dependent children, custody papers for all claimed children (if not living with both natural parents)
- Veteran's Death Certificate, if applicable
- Current Photo I.D. of applicant
- All household income for the past 30 days must be reported for all persons living in the household. This includes all pay stubs, VA Pensions, Social Security income, SSI, Food Stamps, Unemployment Compensation, odd job income, interest income, child support, or income of any other kind
Note: If you are unemployed at the time of the application, you must provide three years of employment history or proof of temporary or permanent disability.
- All bills and expenses for the past 30 days must be reported. Receipts must be provided for any claimed bills or expenses, whether they have been paid or are still outstanding.
- A Mortgage Company Statement must be submitted if you are applying for mortgage assistance. All items on the statement must be completed.
- A Landlord Statement must be submitted if you are applying for assistance and do not own your own home, even if rent or lease assistance is not being requested. All items on the form must be completed.
- A Montgomery County Request for Taxpayer Identification and Certification Form, completed by your mortgage company or landlord, must be submitted if you are applying for rent or mortgage assistance.
Without all documentation, your application will not be accepted.
You will need a Landlord Statement and a Request for Taxpayer Identification form* if you are requesting Rental Assistance.
You will need a Mortgage Company Statement and a Request for Taxpayer Identification form* if you are requesting Mortgage Assistance.
Note: Both forms will need to be filled out by your Landlord/Mortgage Company depending on what you are applying for. This is all their information.
