REQUEST FOR QUOTES / QUALIFICATIONS (RFQ)
The Montgomery County Department of Job and Family Services (MCDJFS) is soliciting Request for Quotes / Qualifications (RFQs) for the following service on the date and time as listed below:
Tuesday, December 1, 2009, no later than 12:00pm Local Time – Quotes and Qualifications are due for –
Non-Emergency Transportation - NET:
To contract with service provider(s) who can provide Non-Emergency Transportation Services to eligible Montgomery County residents.
Letter of Intent due by November 23, 2009 , no later than 4:00pm Local Time
Bidder Project Requirements:
All Bidders of services specified in this RFQ must:
- Have a history of providing curb to curb transportation services to adults and children with variable medical needs and/or handicaps.
- Demonstrate an in-depth understanding and ability of how to provide these services effectively with the designated target population.
- Have the organizational capacity to provide the proposed services immediately following contract approval.
- Have the necessary licensures, certifications, and insurances to publicly operate a transportation service / livery service / taxi service in the state of Ohio.
Target Population:
All customers will be referred by Montgomery County Department of Job and Family Services (MCDJFS) and have been determined eligible for services. Services are to be provided to clients of all ages that are Medicaid eligible, with a variety of medical needs or disabilities, to and from Medicaid reimbursable medical appointments.
A contract(s) awarded as a result of this process shall be effective January 1, 2010 and terminate December 31, 2010 and is subject to State and Federal regulations, including Title VI of the Civil Rights Act and Section 504 of the Rehabilitation Act.
