| Criteria for Ohio Birth Certificate After a Foreign Adoption |
- Parent(s) who adopted a child in a foreign country request the Court to issue an Order for an Ohio birth record (IR3 Visa)
- Original foreign adoption decree or certificate of adoption, foreign birth record and any other documents relative to the adoption along with CERTIFIED TRANSLATIONS must be submitted to the Court for authentication and will be returned
- Passports (parent(s) and child)
- Certified marriage certificate
