- Certified copies of the minor child's birth certificate, death certificate(s) of parent(s), divorce decrees and marriage certificates
(Note: If adoption follows a legal placement, these documents will already have been filed with the legal placement application)
- Petitioner(s) must be available for interview and for a home call prior to the date of hearing
- Everyone in the household 18 years of age or older must have a criminal background check. Please contact the Court for costs.
- On the date set for hearing, the petitioner(s) and child(ren) must appear in Court
- In a grandparent adoption, this Court requires a certified birth record of natural parent(s)
